The State of Florida requires that all volunteers fill out an application so that if an unforeseen accident were to occur, they would have your emergency contact information, state liability protection (Section 768.28, F.S.) and workers compensation (Chapter 440, F.S.).

As the group leader of this event, the state requires that we fill out a Group Volunteer Application. Could you please copy the template below and email it to us at oletatrailblazers@gmail.com
Subject, Volunteer Application Form Are you 18 years of age or older? Participants Name, Email, Phone, Emergency Contact, Contact Name, Contact Phone, Relationship, If you are under 18 years of age, we would need your parent or guardian permission. Please have them email us.
